Facility Evaluation Report
On 03/06/2026 at 9:00 AM, Licensing Program Analyst (LPA) Shakaricka Hughes arrived at the facility to conduct an unannounced annual inspection. LPA Hughes met with the Facility Director Vean. The current census is 63 with 18 facility staff present.
This facility is a single story building licensed to serve (65) ambulatory and (10) non-ambulatory clients . LPA inspected the physical plant including but not limited to the (2) client classrooms, restrooms, kitchen area, and outside perimeter of the facility to ensure compliance with Title 22 regulations. LPA observed the facility to be free of odor, clean and in good repair.
Hot water temperature was measured at 105.8 degrees Fahrenheit in resident bathroom sink, which is within the required regulation of 105 to 120 degrees Fahrenheit. Smoke and carbon monoxide detectors are in compliance with fire safety. LPA observed (3) fire extinguishers located in the facility last serviced on 12/06/2024, not in compliance with fire safety as the service tags were observed outdated. LPA observed the facility has a public telephone in the staff room and the facility has the required posters posted. Facility thermostat was observed at 69 degrees Fahrenheit. LPA observed toxins located in the storage closet kept locked and inaccessible to residents. LPA observed sharp knives kept locked in the staff room kept locked and inaccessible to residents. LPA reviewed medications for 1 client and the medication administration record (MAR) was complete. The first aid kit was checked and contained the required components.
