Facility Evaluation Report

Licensing Program Analysts (LPAs) Vincent Moleski and Jason Lund arrived unannounced on Thursday, March 9, 2023 to conduct an annual inspection. LPAs Moleski and Lund met with program director Vean Loeun and explained the purpose of the visit. The facility is licensed for a capacity of 50 clients, of which 10 may be non-ambulatory.

LPAs Moleski and Lund reviewed staff files for S1, S2, S3, S4. and S5, and client files for C1, C2, C3, C4, and C5.

LPAs Moleski and Lund toured the grounds with Loeun. LPAs Moleski and Lund observed the classrooms, common areas, office areas, medication storage areas, and multipurpose room. The grounds appeared clean and well-maintained. Fixtures and furnishings in all areas were appropriate to meet the needs of clients. Fire extinguishers, first aid kits, and smoke/carbon detectors were observed. Hot water temperatures were measured and observed to be below 120 degrees, as required.

No deficiencies were cited during this visit. An exit interview was held and a copy of this report was left with Loeun.
