The inspector’s account

Licensing Program Analyst Arvin Villanueva (LPA) conducted an unannounced Case Management - Annual Continuation visit today, 8/15/2025, at the facility to continue with the Annual visit initiated on 8/14/2025. LPA met with staff on duty (S1) and explained the purpose of the visit. The Administrator Amor Hernandez is not available to be present for this visit and gave permission to S1 to assist with the visit and sign this report.

During this visit, clients in care were out in the community.

The LPA continued with facility visit to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations.

LPA reviewed 4 staff files, including criminal record clearances. A review of staff records indicates that all 4 facility staff or other individuals who require caregiver background checks are fingerprint cleared and associated to the facility. LPA reviewed 2 of 4 client files. Per review of 1 of 2 client's (R1) medical assessment (LIC602), R1 was assessed as non-ambulatory. However, further review of R1's admission agreement, Alta Califoria Regional Center have assessed R1 to be ambulatory. Per interview with S1, R1 is able to ambulate, do not use assistive devices to ambulate and able to follow staff direction. Advisory was provided to Administrator for R1 to be reassessed for ambulatory status.

Medication review was conducted for 2 of 4 clients. P&I money review was conducted for 2 of 4 clients.

Facility conducts monthly fire drill; last drill was conducted on 7/21/25. Fire extinguisher was observed and last serviced on 10/31/2024. Hot water temperature was measured at 107 degrees Fahrenheit.

Documents requested to be submitted: current LIC500, LIC308, Surety Bond, & Liability Insurance Certificate.

Per the California Code of Regulations, Title 22, Division 6, Chapter 6, no deficiencies are being cited today. Exit interview was conducted and a copy of the report was provided upon exit.
