Facility Evaluation Report

On 7/9/2026, at 2: 20 pm Licensing Program Analysts (LPAs) Reza Jamaly and Christina Valerio arrived unannounced at the facility to conduct the required annual inspection. LPAs met licensee Marilyn Dela Cruz and explained the purpose of the visit, assisted LPAs touring to the facility.

Facility is cleared for 4 resident but there was only one resident (R1)at this time.

LPAs inspected the physical plant including but not limited to the common area, kitchen, dining area, resident bedroom, resident bathrooms, laundry room and outside courtyards of the facility to ensure compliance with Title 22 regulations. LPAs observed the facility to be free of odor, clean and in good repair. LPAs observed bedrooms to be properly furnished with appropriate bedding and lighting. There are no bodies of water present.

LPAs inspected the kitchen and observed a sufficient supply of at least a seven-day supply of nonperishable food and a two-day supply of perishable food. The hot water temperature measured 119 degree F at the resident bathroom sink, which is within the required range of 105°F to 120°F. Grab bars and non-slip mats were observed to be secure and in good condition. Smoke detectors and carbon monoxide detectors were found at the facility. Two fire extinguisher, located in living room and in hallway, was last serviced on 11/25/2025.

LPAs observed that the facility maintained a public telephone in the living room and that all required licensing posters were properly displayed. The indoor temperature measured 72 degree F. Cleaning supplies and other toxic substances were observed to be stored in a locked area and were inaccessible to resident. Sharp knives were secured in a locked kitchen cabinet and inaccessible to resident.
