Facility Evaluation Report

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct an inspection related to a complaint. LPA initially met with Danielle Fabyunkey Lawrence, caregiver, and then met with Anita Heydon, Administrator and Steve Heydon, caregiver who arrived shortly after LPA did.

While reviewing staff files, LPA observed there was not any documentation related to staff training for staff (S1 and S2) and the last training documentation on file for staff (S3) was from 2021.

Per Health and Safety Code § 1569.69 and § 1569.626, the following (2) citations are being issued.
