The inspector’s account

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a case management inspection and met with Elisabeth Lucas, lead staff, stating the reason for the inspection. Also present was Teodoro Lucas, DSP. LPA observed (3) residents in the common area and was advised (1) resident was attending day program. LPA observed Staff E. Lucas, to be ambulating with resident (R1) in the common area, due to their fall risk. The administrator was notified, by phone, that LPA was present to follow up after receiving an incident report for (R1), who went to the Emergency Room on July 30, 2026, around 4:45 pm.

LPA and E. Lucas discussed the incident report, submitted to the Department earlier today, about (R1) being sent to the Emergency Room after displaying shaking symptoms, appearing to possibly be related to a seizure. The Emergency Room physician contacted the Administrator (10:00 pm) to advise that (R1) had a Urinary Tract Infection (UTI). (R1) was prescribed the antibiotic, Cefpodoxime 200 mg tablet, to be taken twice daily for (10) days, is back to their baseline, walking in the common area and sleeping on the couch. The facility will take (R1) to their follow up appointment on August 7, 2026, with their primary care physician.

LPA reviewed hospital discharge paperwork which lists the 10-day antibiotic prescription for Cefpodoxime 200 mg. The paperwork also provides care instructions for a seizure. Staff E. Lucas confirmed (R1) does not regularly get UTI's and continues to require full assistance with toileting. LPA spoke to the Administrator who was at the pharmacy picking up (R1's) antibiotic prescription and confirmed (R1) will receive the first dosage this afternoon. The Administrator stated the physician indicated a UTI could cause a seizure.

The facility took appropriate action in sending (R1) out for further medical attention upon seeing a change in condition. There are no deficiencies issued in this report.
