Facility Evaluation Report
LPA Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a case management inspection and met with DSP, Milagros "Mila" Andres. LPA was informed that client (C1) was out of the community attending a scheduled medical eye appointment and laboratory work with Lead DSP, Elvie Nitollama. DSP Mila stated that only client (C2) was currently at the community and was resting in their room and (4) clients were attending day program. DSP Aurelio Andres arrived around 11:00 am.
LPA stated the reason for the inspection was to follow up on an incident report submitted to the Department on June 23, 2026, relating to client (C1) being sent to the Emergency Room due to trouble breathing. LPA and DSP discussed the incident report and reviewed documentation for recent medical appointments/phone calls, detailed charting notes, and medication records. (C1) attended a scheduled medical appointment on June 15, 2026 and there were changes related to when to take the medication, Spironolactone 25 mg and PRN Dextromethorphan 10-100 mg/5 ml, and was related to the diagnoses, including asthma and a cough. The MAR shows the medication changes were made and the medication was given as ordered.
LPA reviewed documentation that (C1) graduated from hospice services around March 20, 2026 and is cleared to resume attending day program. DSP Aurelio stated that (C1) attended day program from mid-end of April 2026, a few days at the end of May, (5) days in June, through June 11, due to breathing issues. DSP stated to LPA that (C1) is currently eating better, was only using oxygen at night, and wants to return to day program. Both DSP's stated (C1) is planning to return to day program soon and is going to get a portable oxygen machine, in case it is needed while at day program.
It appears the facility took appropriate steps in sending (C1) out for emergency medical treatment on June 23, 2026 and continues to monitor and take (C1) to regular appointments. There are no deficiencies cited. Exit interview. Copy of report provided.
