Facility Evaluation Report

Licensing Program Analyst (LPA Sabrina Calzada arrived at the facility, at 3:00 pm, and met with Administrator, Maye Dickey. Due to the facility being currently remodeled and no clients on site, a scheduled time was set to do a brief walk thru to conduct the required annual due.

LPA conducted a walk-through with the Administrator to confirm there are currently no clients. There have not been any clients since on/around February 2025. Administrator showed LPA how each bathroom and kitchen have been remodeled, and there will be new flooring throughout.

Outside the front entrance in the driveway area. LPA observed construction vehicles and materials, including soil and paint. The backyard is being redone also with a new gazebo.

The Administrator stated the remodeling project is expected to be completed on/around October 2025, and will notify the Department and the Regional Center when the clients will return to the home. The Administrator stated the home may be closed and re-opened under a new license.

There are no deficiencies being cited in this report.
