Facility Evaluation Report
Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a required annual and met with Lita Cailing, Co-Administrator, and explained the purpose of the inspection. Initially, (1) client was present. LPA was advised (5) clients were attending day program. The facility is an Adult Residential Facility (level 3 home), vendorized through Alta California Regional Center, and approved for (6) non-ambulatory clients. LPA observed (3) clients return to the community during the inspection.
LPA and Co-Administrator toured the interior/exterior of the facility including the common areas, (4) private client bedrooms, (1) shared client bedroom, (2) bathrooms, kitchen, (2) staff rooms, laundry area and outside patio. LPA observed the home to be clean, odor-free, in good repair, and to not pose a health and safety risk or personal rights violation. Toxins and sharps in locked in the laundry area and medications are locked in a separate cabinet. There are additional toxins locked under the kitchen sink. There is sufficient 2+day perishable, including fresh produce, and 7+day non-perishable food. Bathrooms have grab bars, skid-resistant flooring and hygiene supplies. There are sufficient PPE supplies and linens/towels and a complete First Aid kits. Hot water measured 110*F in the kitchen. Smoke/monoxide alarms are working and the fire extinguisher was last serviced June 29, 2026. Emergency drills are conducted monthly and documented. There is an unlocked exit and patio seating with shade. Exit doors have alarms.
LPA reviewed (3) client files and (3) staff files. Files are very organized and contain current documentation. Medications were reviewed for (2) clients-documentation is current and no discrepancies were noted. P&I records are maintained. Staff training is current, including First Aid/CPR. Administrator certificates #7036197735 (exp 11/25/26) and #7036196735 (exp 7/10/27) are posted in the common area with other required postings. There were no deficiencies observed. Exit interview.
