On Aug 12, 2025 (8/12/25), Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management and met with assistant administrator .
On 8/11/25 the department received an incident report from the facility regarding R1. R1 reportedly made statements alleging actions on the part of a caregiver (S1).
Licensee reported the allegation as required.
During this visit LPA met with R1 and a family member, interviewed assistant administrator and licensee.
LPA requested records regarding R1 and S1. Records to be submitted by 8/13/25.
As a result of today’s inspection, no deficiencies were noted at this time.
