The inspector’s account

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a case management inspection to discuss (2) incident reports submitted to the Department earlier in July 2026. LPA met with Chelsea Naruse, Administrator, and stated the reason for the inspection. LPA observed (3) clients in the common areas and (2) clients (C1/C2) return from day program shortly after arriving. There are currently (6) residents residing in the community. Also present was DSP's Cheryl Dwyer and Graciella Wooder.

After returning from day program on Jun 24, 2026, client (C1) vomited (30) minutes after eating , their temperature began increasing and was taken to the Emergency Room for further evaluation. After some tests and an X-ray were completed, (C1) was admitted to the hospital for signs of pneumonia and discharged on July 7, 2026, after regaining the ability to walk without a walker. (C1) attended a follow up appointment with their primary care physician, who made no medication changes, and was given a referral to a gastrologist, which is pending due to trying to obtain a physician/hospital that will be covered by (C1's) insurance. (C1) has returned to day program and is back to baseline. The administrator stated (C1) likely aspirated while eating lunch at day program, causing Pneumonia. The hospital discharge paperwork notes (C1) requires/returned to a "mechanical soft" diet.

Client (C2) was sent to the Emergency Room, on July 9, 2026, while attending day program after suddenly crying and wheezing, and stating their body hurts. (C2) was transported to day program by an outside company, who checked (C2's) temperature prior to boarding the bus, and it was normal. (C2) returned to the care home on July 12, 2026 and will follow up with a nephrologist. (C2) uses incontinent briefs which could have caused the Acute Urinary Tract Infection (UTI) and Sepsis they were diagnosed with.

The facility took appropriate action in advising the day program and/or seeking emergency medical attention for both (C1/C2). There are no deficiencies issued in this report . Exit interview. Copy of report provided.
