Facility Evaluation Report

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to deliver complaint findings and met with staff, Jecca Keifert and Cheryl Dwyer. LPA observed (2) clients were present at the start of the inspection and (4) clients returned from day program during the inspection. The Licensee, Nimfa Segovia, Licensee, arrived around 3:10 pm, and Chelsea Naruse, Administrator, arrived around 3:30 pm.

The purpose of this report is to issue a citation that was discovered during the course of complaint investigation #59-AS-20251017094929 .

During the course of the investigation, it was discovered that the facility reported to the Regional Center but did not report to the Department when client (C2) eloped from the facility on May 25, 2025.

Although (C2's) physician's report (July 2025) notes that (C2) can leave the facility unassisted, the facility does not usually allow (C2) to leave independently due to (C2's) diagnosis and age.

Per California Code of Regulation Title 22, Chapter 6, Division 8, the following (1) citation is being issued in this report on the 809-D page.
