Facility Evaluation Report

Licensing Program Analyst (LPA) Sabrina Calzada arrived for a scheduled inspection at 01:30 pm and met with Daniel Segovia, Assistant Administrator. The Department received a written facility closure notice from the Licensee, by email, today, 6/11/25, requesting the facility license be closed due to personal reasons. The facility has been non-operational for several years.

LPA and Assistant Administrator toured the facility to ensure there are no residents present.

The facility license was obtained today by the LPA.

The Administrator indicated that Alta California Regional Center has been notified of the pending facility closure.

There are no deficiencies issued in this report.
