Facility Evaluation Report

Licensing Program Analyst (LPA) Sabrina Calzada arrived for a scheduled inspection at 01:30 pm and met with Daniel Segovia, Assistant Administrator. The Department received a written facility closure notice by email, today, 6/11/25, that the facility is closing due to personal reasons. The facility has been non-operational for several years.

This document is being created to clear the annual in the system. A separate report will be created today to document the facility is to be closed, as requested by the Licensee, Nimfa Segovia.

LPA and Administrator toured the facility to ensure there are no residents present.

The facility license was obtained today by the LPA. The facility license fees (due August 9,2025) will not be paid since the facility is closing effective today, June 11, 2025.

There are no deficiencies issued in this report.
