Facility Evaluation Report

Licensing Program Analyst (LPA) Sabrina Calzada arrived for a scheduled inspection at 03:10 pm and met with Daniel Segovia, Assistant Administrator. The Department received a written facility closure notice from the Licensee, by email, on July 25, 2025, requesting the facility license be closed due to personal reasons.

All residents (3) were relocated to other homes in the area on/around 8/1/25.

LPA and Assistant Administrator toured the facility to ensure there are no residents present.

The facility license was obtained today by the LPA.

Alta California Regional Center has been notified and was involved in all clients relocating.

There are no deficiencies issued in this report.
