The inspector’s account

Licensing Program Analyst (LPA) Sabrina Calzada arrived announced to conduct a required closure inspection and met with Kim Rosa, House Manager.

LPA and the House Manager conducted a tour through the interior of the facility. LPA did not observe any residents present. Furniture was still in place in resident rooms and common areas.

Both the Licensee and House Manger confirmed that all (3) residents moved out by Monday, June 22, 2026. (2) of the residents relocated to a related facility in the area and (1) relocated to another facility in the nearby area.

LPA obtained the facility license from the House Manager and explained the license will be closed effective June 26, 2026.

There are no deficiencies issued in this report.
