Facility Evaluation Report
Licensing Program Analyst (LPA) Bethany Mirlohi arrived unannounced to conduct a case management visit. LPA met with administrator, Amanda Britt, during today's inspection. LPA completed required COVID-19 testing protocols, and a daily self-screening questionnaire for symptoms of COVID-19 infection to affirm no COVID-19 related symptoms. LPA ensured they applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: Masks.
LPA arrived today to discuss recent incident reports received. On 5/11/22, C1 and C2 were in a physical altercation and the police department were called. C1 has a court hearing in a few months to address the incident. Paramedics arrived and performed first aid on C1 and C2, and both clients remained in the home. LPA reviewed with administrator the interventions that are in place for all clients in care. LPA reviewed staff interventions and training. Administrator reported incident to CCL, regional center and responsible parties.
At this time no deficiencies are being cited. Exit interview conducted.
