The inspector’s account

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct an annual inspection and met with Tyler Moorman, Director, and Juan Velasquez, owner, at the start of the inspection. LPA stated the reason for the inspection. The location is a licensed Adult Day Program (ADP), vendorized through the regional center, and operates on a 3:1 staff ratio. The facility is licensed for (55) ambulatory and (5) non-ambulatory clients. Today, there are fifteen (15) clients, (6) staff and (3) Associate Directors present. (2) of the clients are non-ambulatory.

LPA and the Director toured the interior/exterior of the facility including the office, kitchen, (6) client classrooms, (3) client bathrooms, isolation room, gardens and patio. LPA observed the facility to be clean, safe and in good repair. There is sufficient indoor and outdoor space for clients to participate in a variety of activities. LPA observed staff present with clients in each classroom and clients to be engaged in different types of activities. All classrooms have a first aid kit and an extinguisher- last serviced 12/19/2025. The facility offers packed snacks only and water bottles. Clients bring their own lunches. Bathrooms have hygiene supplies. Outside, there are gardens, seating and space for activities. The facility takes clients on regular community outings, including to the park. Staff and clients participate in disaster drills every two months. There is a recycling program on site. Hot water was below 105*F- the owner stated local companies were contacted for annual servicing on the water heater. Facility vehicle receives regular maintenance.

LPA reviewed (4) client files and (4) staff files. Files were organized and contained required documentation. Administrator and staff are completing more than the required training each year, including "hands-off" for Pro-Act and First Aid/CPR. All staff are cleared/associated. There are no citations issued in this report.
