Facility Evaluation Report

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to deliver an amended return from a prior inspection on May 8, 2026. LPA met with Steven Ronstadt, Administrator, and stated the reason for the inspection. Also present were Administrator Designee, Lou-Anne, and staff, Shadae White.

LPA observed resident (R1) in the common area during today's inspection. LPA and the administrator spoke to a family member who was visiting (R1). This family member provided more background information on (R1) and their diagnosis. Additionally, LPA spoke to a second family member, by phone, today to provided additional background information on why (R1) is not generally comfortable speaking with someone she just met and the door closed. The second family expressed that (R1) has been the best they have been in years since moving to this facility.

The administrator updated LPA on the facility's staffing.

LPA observed (3) residents resting in the common area and (2) residents in their rooms. LPA was advised (1) resident was still on an outing.

There are no deficiencies issued in this report.
