The inspector’s account

(LPA) Sabrina Calzada and Ombudsman, Melissa Basler, arrived unannounced to conduct a case management inspection. LPA met with Kelly Conley, House Manager, and stated the reason for today's inspection. Also present were staff Mishka Smith and Shadae White.

During today's inspection, LPA, the Ombudsman and House Manager discussed concerns that arose during a prior visit with the Administrator and another Ombudsman on April 8, 2026. The Administrator joined the discussion by phone at 10:45 am. A lengthy discussion was then held regarding the facility's concerns about an Ombudsman who had visited the facility for the first time speaking privately to residents, as well as the Ombudsman not feeling supported by the Administrator during their visit. Both sides offered solutions to prevent a similar type occurrence from happening again.

Also discussed were specific concerns brought to the Ombudsman attention on April 8, 2026 relating to residents (R1 and R2). The House Manager provided detailed background information on each resident which helped clarify and dismiss any concerns previously raised.

LPA, the Ombudsman and House Manager spoke to resident (R1) who was resting in their room. (R2) was sent to the hospital on May 6, 2026 due to possible change in condition, including being lethargic, disoriented, shaky and not able to stand. (R2) is expected to return to the community in a few days.

LPA observed (5) residents resting in their rooms and did not observe health and safety risk to the clients.
