Facility Evaluation Report

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a health and safety inspection and to follow up on a discussion with the Administrator yesterday by phone. LPA met with Claudia Mihai, Administrator, and stated the reason for today's inspection. Also present was culinary staff, Nana, who was helping with food preparation today. LPA observed (1) resident in the common area watching television and (4) residents in their rooms resting and was advised (1) resident was currently attending a group session. Administrator, Claudia Mihai, informed LPA that (2) new residents moved in today. Currently, (2) residents are on hospice. During the inspection, the pending Licensee arrived to check on the facility.

LPA and Claudia Mihai, toured the interior and exterior of the facility. All rooms were observed to be clean, in good repair and odor free, and there is a covered patio area outside with seating and exercise equipment. Inside temperature measured 74*F.

LPA and the Administrator discussed how RCFE Administrator certificate #6052833740 (exp 7/31/2023) was recently submitted for renewal, with late fees paid, and received by the Department on June 18, 2025. Department records show RCFE Administrator certificate #7019737740 is pending renewal. The Administrator provided LPA with documentation by email, on 6/30/2025, showing the required training to recertify as an Administrator was completed in March 2025, after she started as an Administrator, in February 2025. There is a pending license as a result of a change in ownership. The current licensee was notified by email on 7/1/2025 that the Administrator RCFE Certificate is currently pending renewal.
