Facility Evaluation Report
Licensing Program Analyst (LPA)Sabrina Calzada arrived unannounced to conduct a required annual and met with the Administrator, Titus Popa, and stated the reason for today's inspection. Also present were staff, Andrew Plesco and Maria Plesco. LPA observed (6) residents in the common area at the start of the inspection. The facility is licensed for (6) non-ambulatory residents and has an approved hospice waiver for (3) residents. Currently there are (0) residents on hospice.
LPA and Administrator toured the interior/exterior of the facility including the common areas, (4) private resident bedrooms (1) shared resident bedroom, (3) resident bathrooms, kitchen, office, staff room and laundry/garage area. The facility was clean, in good repair and odor-free, and the bathrooms have the necessary grab bars, skid-resistant flooring, paper towels and hand-washing posters. There is sufficient 2+day perishable, including fresh produce, and 7+day non-perishable supply of food. Sharps are locked in the kitchen, medications are locked in a nearby cabinet, and toxins are locked in the laundry room. Inside temperature measured 75*F and hot water measured 118*F in a bathroom. Fire extinguisher was last serviced on 12/10/2025, and the smoke alarms are functioning. There are activities/games on site with sufficient indoor/outdoor space, including a walking track and covered patio area with seating. There are (2) unlocked exit gates and no pools. There are no blockages near exit doors and alarms are working. Annual fees are current as well as Secretary of State filing. All required postings are visible in the common area.
LPA reviewed (2) resident and (8) staff files. Files are organized and contain current documentation, including care plans. Staff are completing annual training,including First Aid/CPR. Administrator RCFE certificate 7007834740 (exp 3/7/27). Administrator Designee also has a current certificate. LPA obtained a current copy of current liability insurance. LIC500 to be emailed by 2/10/26.
There were no deficiencies observed. Exit interview. Copy of report provided.
