Facility Evaluation Report

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a required annual and met with Administrator, Titus Popa. LPA stated the reason for today's inspection. LPA observed (3) residents in the common area and (3) residents in their rooms during the inspection. Also present were staff, Ted Popa and Valerie Popa. The facility is licensed for (6) non-ambulatory residents and has an approved hospice waiver for (4) residents. Currently there are (0) residents on hospice.

LPA and Administrator toured the interior/exterior of the facility including the common areas, (4) private resident bedrooms (1) shared resident bedroom, (3) resident bathrooms, kitchen, office, staff rooms and laundry/garage area. The laundry/garage and staff room area are always locked. The facility was clean, in good repair and odor-free. Bathrooms have the necessary grab bars, skid-resistant flooring, and hygiene supplies. There is sufficient 2+day perishable, including fresh produce, and 7+day non-perishable supply of food. Sharps are locked in the kitchen and medications are secured nearby. Toxins are locked in the laundry area. Inside temperature measured 76*F and hot water measured 116* in a resident bathroom. Fire extinguisher was last serviced on 12/10/2025, and the smoke alarms are functioning. All exit door alarms are working, and there are no blockages. Staff are aware where the utility shut-off valves are. There is sufficient indoor/outdoor space, including a walking track on front patio and covered seating on back patio. There is (1) unlocked exit gate and (1) open gate. All required postings are visible in the common area.

Three (3) resident files and (8) staff files were reviewed . Files are organized, and with current documentation, including training certifications. Administrator has current RCFE Administrator certificate #7007834740 (exp 3/7/27), and the Administrator Designee also has a current certificate (exp 3/1/27).

LPA obtained a current copy of current liability insurance. LIC500 to be emailed by 2/10/26.

There were no deficiencies observed. Exit interview. Copy of report to be emailed.
