The inspector’s account

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a case management inspection and met with Analou Zabala, DSP. LPA stated the reason for today's inspection and spoke to Licensee, Jowella Chico, by phone, who arrived around 2:00 pm. The purpose of today's inspection was to follow up on an incident report submitted to the Department on February 7, 2026. LPA observed (3) clients return from day program at 3:00 pm and was advised client (C1) remains hospitalized from two days ago.

LPA, the DSP and Licensee discussed the most recent incident as well as multiple prior instances last Spring 2025 when (C1) called the crisis center to be sent to the hospital. The administrator explained that (C1) has shown this pattern of behavior for the last three to four years, and the behavior is specifically seen from February through May of each year. LPA reviewed (C1's) binder and observed (5) incident reports from March through May and (1) incident report from September. LPA reviewed discharge paperwork also.

The administrator stated (C1) has an appointment with their primary care physician on February 11, 2026 and two appointment scheduled on February 12, 2026. One appointment is with (C1's) Advanced Psychiatry and the second appointment is with the Conservator, Behavioral Consultant, Service Coordinator, day program. The facility Licensee will attend all meetings and will provide an update to the Department following the meetings. It was discussed how (C1) often calls the crisis center in the afternoon, typically after returning from day program. Additionally, (C1) is observed to be more agitated and emotional prior to calling. This information will be addressed more in detail during the appointments this week. The Licensee indicated that since there is now a pattern of three of more years, the behavior may able to be diagnosed.
