Facility Evaluation Report
Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to complete the required annual that was started earlier this month. The annual inspection was not able to be completely finished due to the main building still being under construction due to water damage detected a few months ago. LPA observed the construction zone signage to have been removed from the main building upon starting the inspection.
LPA met with Maria Rope Chico and Ana Lou Zabala, DSP's, and explained purpose of inspection. Also present was Angelo Chico, DSP. Jowella Chico, Licensee and Cristina Chico, Administrator, arrived at 2:10 pm. The facility is a licensed 4I - Prader Willi home that is vendorized through Alta California Regional Center for (6) ambulatory clients only. LPA observed (2) clients to be working on an art project, (2) clients to be in their rooms and (1) client returned from day program. The Administrator and DSP indicated that they just started moving all the clients' belongings back to the main building yesterday. LPA conducted a tour and observed all clients' bedroom furniture and belongings back. Only kitchen stuff and staff files remain to be moved back. LPA observed newly installed flooring and dry wall that was painted during the repair work. LPA observed sufficient 2+/7+day food on site, including fresh produce. Sharps, medications and toxins to be locked.
LPA observed the Fire extinguisher to have been last serviced 10/26/23. LPA reviewed (3) staffing files and found them to be organized and contain current training documentation, including First Aid/CPR. Administrator Certificate current- # 6036853735- exp 1/17/25. LPA reviewed the Mitigation Plan and advised the facility to complete Infection Control Plan (LIC9282) which is the most updated form- Licensee advised to submit the completed LIC9282 by 11/3/23. LPA reviewed the Emergency Disaster Plan and found it to be complete.
There are no deficiencies cited during today's inspection.
