The inspector’s account

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a required annual. LPA met with Ana Lou Zabala, Maria Lina and Maria Rope Chico, DSP's, and explained purpose of inspection. Also present was Angelo Chico, DSP. LPA spoke with Jowell Chico, Licensee, by phone, who arrived at 1:15 pm. The facility is a licensed 4I - Prader Willi home that is vendorized through Alta California Regional Center for (6) ambulatory clients only. LPA observed (5) of (5) clients in the common area. The property is enclosed by a vehicle gate that operates manually. The gate is not locked.

LPA observed warning signs outside the facility that it is still being repaired from a sewer flood and water damage that occurred in one of the client bathrooms in July 2023. LPA was advised the construction work is in the final stages and will be completed in the next week or two. The construction was reported to the Department and to the Regional Center in July 2023 and clients were moved across the driveway to a recently remodeled 2.5 bedroom/1 bathroom residence with a full kitchen. The facility has retained access to the laundry area (2) resident rooms where clients could get belongings, during the construction. LPA toured the interior and exterior of the newly remodeled building (across the driveway) and observed it to be clean, in good repair and odor free. LPA observed locked toxins, medications and sharps and sufficient 2+day perishable/7+day non-perishable food. Hot water temperature measured 116*F in the kitchen. The food and refrigerator are locked due to clients having Prader-Willi Syndrome. LPA observed paper towels, soap, and trash can in the bathroom. There are games/activities on site. Client/Staff interviews conducted.

LPA reviewed (3) client files and found them to be organized, complete and contain current documentation. Medications were reviewed for (2) clients- no discrepancies were noted and the facility correctly uses a Medication Administration Record for scheduled and PRN meds. P&I funds were reviewed for (2) clients and there were no errors noted. Documentation is current. LPA will return later this month and complete the annual will be completed when notified the construction has been completed. There were no deficiencies observed today. Exit interview. Copy of report provided to the Licensee.
