Facility Evaluation Report
Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a required annual and met with Josie Chico, staff. Also present was Maria Lina Santa Ana, who began preparing food in the kitchen. The facility is a licensed 6 Prader Willi home that is vendorized through Alta California Regional Center for (6) ambulatory clients only. LPA was advised all (3) clients were currently attending day program and would return around 3:00 pm. There are vehicle/pedestrian gates at the front of the property -both gates were open/unlocked when LPA arrived.
LPA and the Administrator toured the interior/exterior of the facility and observed it to be clean, in good repair and odor free. LPA observed each client to have their private bedroom. Each resident room has a locking mechanism on the door; all clients have their own key. Bathrooms (3) are clean and supplied with hygiene products. LPA observed locked medications and sharps and sufficient 2+day perishable/7+day non-perishable food. The food and refrigerator are locked due to clients having Prader-Willi Syndrome. Hot water temperature measured 110*F in the kitchen. Fire extinguisher was last serviced 10/14/25, and (3) smoke/monoxide alarms were tested and working. There are sufficient linens/towels/paper products, and a complete First Aid kit. There are games/activities on site, and sufficient outdoor space for activities. All toxins are locked in the laundry area. Required postings are visible in the common area.
LPA reviewed (2) client files and (3) staff files. Files are organized, complete and contain current documentation. P&I records and receipts are maintained. Medications/orders were reviewed for (1) client-no discrepancies noted. Administrator Certificate current- # 7010781735 (exp 1/17/27).
