The inspector’s account

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a required annual inspection and met with Adina Sbingu, Administrator. Also present was staff, Tamara Grant. LPA observed (2) residents watching television and (2) residents resting in their rooms. The facility is licensed for (6) non-ambulatory residents, (1) or whom may be bedridden. There is a hospice waiver for (2)- currently no residents are under hospice care. (1) resident returned from day program at the end of the inspection.

LPA and Administrator toured the interior/exterior of the facility including the common areas, (6) private resident bedrooms, (3) resident bathrooms, staff room, kitchen, laundry and garage. LPA observed the facility to be clean, in good repair and without any odors. All bathrooms have the necessary grab bars, non-skid flooring, and hygiene supplies. There is sufficient 2+ day perishable, including fresh produce, and 7+day non-perishable supply of food. Sharps are locked in the kitchen, toxins are locked in the laundry room and medications are secured in a cabinet. Hot water measured 125*F in the kitchen- sign posted. Inside temperature measured 72*F. Fire extinguishers were last serviced on 6/24/2025, and smoke/monoxide alarms are functioning, including the automatic fire door. Residents use a call button that has a monitoring device in the kitchen and each staff wear. There are sufficient linens/blankets/towels/paper products and PPE supplies and games/activities, All exit doors have alarms. There are no pools or fountains. Screens are in good condition. Discussed Guardian roster updates, upcoming training webinar and other topics.

LPA reviewed (2) staff and (2) resident files. Files were organized and contained current documents. Medication orders were reviewed for (2) residents. Medications are being administered per orders. Staff has completed initial training and will complete continuing training this month. First Aid/CPR are current. RCFE Administrator certificate #7035965740 (exp 11/2/27). There are no citations issued in this report.
