Facility Evaluation Report

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a required annual inspection and met with Adina Sbingu, Administrator. Also present was staff, Tamara Grant. LPA observed (2) residents watching television and (3) residents resting in their rooms. The facility is licensed for (6) non-ambulatory residents, (1) or whom may be bedridden. There is a hospice waiver for (2)- currently no residents are under hospice care.

This report is being generated to clear the Post-Licensing inspection in the system.
