Facility Evaluation Report
Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a required annual and met with Sione Finau, Administrator Designee. The Administrator, Heta Finau, was out of town but spoke to LPA by phone. All (3) clients were attending day program at the start of the inspection but returned from day program around 3:00 pm. The facility is a level- 2 home, vendorized by Alta Cailfornia Regional Center and is licensed for (4) ambulatory clients. All clients are ambulatory.
LPA and Administrator Designee toured the interior/exterior of the facility including the common areas, (2) client bedrooms, (2) client bathrooms, kitchen, and laundry area. LPA observed the home to be clean, safe and in good repair. Each client door has a locking mechanism, as required by the Regional Center (Title 17). Inside temperature was observed to be 80*. Facility conducts monthly fire drills- last drill was 9/21/25 and clients participated also. Smoke/monoxide alarms are in working order. Fire extinguisher was last serviced 10/11/24- facility agrees to have the extinguisher re-serviced by 10/11/25. Hot water temperature measured 107*F in the kitchen. LPA observed sufficient 2+ day perishable and 7+day non-perishable supply of food- there is an additional freezer in the garage. LPA observed locked medications in the kitchen. There are sufficient towels/linens/blankets and paper products. There is a table with chairs and umbrella in the backyard and (1) exit with a gate. Facility to install a locking mechanism on the gate that can be opened from the inside but is locked from the outside, if they wish to lock it. Annual fees have been paid.
LPA reviewed (2) staff files and (3) client files. Files were organized and contained current documentation, including for required training. P&I funds and medications were reviewed for (2) clients - records match funds on hand and medication documentation is current. Administrator's certificate has been renewed- #7001133735- exp 7/23/27. There are no deficiencies cited in this report. Exit interview. Report provided.
