Facility Evaluation Report
On 05/21/2025, Licensing Program Analyst (LPA) Cheyenne Ratajczak and Licensing Program Manager (LPM) Laura Munoz arrived at the facility unannounced to conduct a Required-1 Year Inspection utilizing the inspection tool. LPA and LPM met with Administrator, Michael Offiah, and explained the purpose of the visit.
LPM and LPA and administrator conducted a tour of the facility. Areas toured include but not limited to resident bedrooms, resident bathrooms, staff room, kitchen, garage, backyard and common areas. During the tour of the facility, LPA and LPM observed facility fire alarm pull system to be inoperable. LPM contacted Sacramento Metro Fire Department who dispatched a fire inspector during the visit. Based on the fire inspector's inspection, the facility fire alarm system as well as 4 smoke alarms were found to be inoperable. Additionally, it was found that a resident's bed in room #3 was found to be blocking a designated exit door.
During today's visit, the following deficiencies were observed:
Facility was pre pouring medications for seven(7) in advance
Some facility windows had missing or broken screens
The resident common bathroom was observed to have rust the in shower and toilet; The cabinet under the sink has a broken door
The exterior of the facility had multiple fence planks missing from street facing fence
Facility common bathroom was missing a door knob
Facility is not documenting residents medications
LPA and LPM will refer the facility to Technical Support Program (TSP)
Deficiencies are being cited during today's inspection. Civil penalties assessed. Exit interview conducted and a copy of the report was left at the facility.
