Facility Evaluation Report
Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a required annual inspection. LPA knocked/rang at the door and there was no answer. LPA reached out to phone contacts on file and left a message for Administrators, Cecilia Pestrana and Donnie Pestrana. LPA received return calls from both noting the Administrator Designee would be at the facility shortly. Maria Pestrana, Administrator Designee and Lead caregiver/DSP, Liza Pestrana, arrived around 1:45 pm. The facility is a level 3 home, licensed by Alta California Regional Center. All (6) clients returned from day program during the inspection.
LPA and Administrator Designee toured the interior/exterior of the facility including the common areas, (3) shared client bedrooms, (2) client bathrooms, kitchen, laundry area/garage, and sun room. LPA observed the home to be clean, safe and in good repair and to not pose a health/safety risk or personal rights violation. Inside temperature measured 74* F. Resident rooms have locks installed per Title 17 and residents can have a key if desired. Fire extinguisher last serviced 2/20/2025. Facility conducts monthly fire drills, at different hours. The facility has a large back yard area with seating and an unlocked exit gate. All toxins are locked in the laundry area and medications are locked in a separate cabinet near the kitchen. There is sufficient 2+day perishable/7+day non-perishable food, and all food is labeled when opened or purchased. Water temperature measured 111* in the kitchen. Smoke/monoxide alarms are working. There are games/activities on site. Required posting are visible in the common area. Administrator's Certificate #7000121735 (pending).
LPA reviewed (3) client files and (4) staff files. Files were organized and contained current required documentation, including care plans and staff training. . Staff have current First Aid/CPR. P&I records were reviewed for (2) clients and medications records for (3) clients. No discrepancies were noted. Surety bond (exp 5/1/2026) is current. LPA requested copy of LIC308/LIC500. There are (2) other active Admin Certificates- valid through Sept/Nov 2025.Infection Control Plan to be filed with Emergency Disaster Plan. There were no discrepancies noted during today's inspection. Exit interview. Copy of report provided to the Administrator.
