Facility Evaluation Report
On 12/09/2025 Licensing Program Analyst (LPA) Cheyenne Ratajczak arrived at the facility unannounced to conduct a case management visit. LPA met with Administrator Jacelyn Monte and explained the purpose of the visit.
During a visit on 11/06/2025, it was observed that the facility garage had been converted into two (2) rooms that are being utilized for staff sleeping quarters. After reviewing facility sketch and fire clearance, it was determined that the facility is not approved to use the garage as living quarters for staff or residents.
The Licensee is advised that staff and/or resident shall not sleep, rest or reside in the unpermitted garage conversion until the area is permitted and approved by appropriate agency. Should the licensee fail to comply, additional citations and/or civil penalties may be assessed.
