On 01/12/2026 an office meeting was held with Licensee. LPM Laura Munoz and LPA Cheyenne Ratajczak met with Licensee Jacelyn Monte to discuss the current operation of the facility. Licensee is working with ALTA California Regional Center regarding current vendorization. The department advised the Licensee that they will schedule a follow up meeting if needed.
Exit interview conducted and a copy of the report emailed to Licensee.
