Facility Evaluation Report

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to deliver an second amended page to a complaint investigation report (# 59-AS-20230504081443) delivered on 9/29/23. LPA discovered earlier today that the information was incorrectly copied/pasted in the prior amended 9099C-2 page. LPA met with Toni Jones, Administrator, and explained purpose of inspection.

LPA provided a copy of the complete complaint findings for the above referenced complaint, which included the most recently amended page (9099C-2), any other amended pages and the original pages that were not amended.
