Facility Evaluation Report
Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a required annual inspection and met with Melody Bulanadi, Co-Administrator/caregiver,stating the reason for today's inspection. Also present was Edgar Bulanadi. LPA observed (2) clients resting in the common area and (1) client in their room. LPA observed (1) client return from day program around 2:30 pm. The facility is a level 4--I home, vendorized by Alta California Regional Center, and is licensed for (4) clients, (3) of whom may be non-ambulatory. Clients are residing in the appropriate room per the fire clearance.
LPA and Co-Administrator toured the interior/exterior of the facility including the common areas, (4) private resident bedrooms, (1) staff room, (2) resident bathrooms, kitchen, laundry area and garage. LPA observed the facility to be clean, in good repair and odor-free and each bathroom to have the necessary grab bars and hygiene supplies. LPA observed sufficient 2+day perishable, including fresh produce, and 7+day non-perishable supply of food. Sharps, toxins and medications are locked in the kitchen. Hot water measured 115.3*F in the kitchen and the freezer/refrigerator measured 0*F/40*F degrees. Smoke/monoxide alarms are working, and the fire extinguisher was last serviced 8/4/25. There are (2) unlocked exit gates and outdoor seating area. There is not a pool.
LPA reviewed (2) of (4) client files and (6) staff files- Files were organized and contained current documentation, including staff training. LPA reviewed P&I records and medications for (2) clients- no discrepancies were noted. PRN is documented. All staff are cleared/associated. ARF Certificate #7004568735 (exp 5/25/26) and Co-Administrator Certificate #7010900735 (exp 5/31/26) -pending renewal. LPA requested an updated copy of the LIC308 and LIC500 be provided to CCLD by 3/18/26. There were no deficiencies observed and no citations issued during today's inspection. Exit interview.
