Facility Evaluation Report
On 07/29/2026, Licensing Program Analyst (LPA) Cheyenne Ratajczak arrived at the facility to conduct a Required-1 Year Inspection. LPA met with Administrator, Anyang Lu, to conduct the visit. There are currently no residents at the facility. Administrator stated they are going to reapply with ALTA for a lower level of care due to the fact they are having a hard time finding a Board Certified Behavior Analyst (BCBA).
LPA and administrator toured the interior and exterior of the facility. Areas toured include but are not limited to: common areas, three (3) residents' bedrooms, bathrooms, kitchen and backyard. LPA observed required furniture, and lighting throughout the residents' bedrooms and facility. LPA observed residents' bathrooms to be clean, sanitary, and in good repair. LPA observed fire detectors and carbon monoxide alarms to be operable. LPA observed required Licensing posters posted throughout the facility. All emergency exits were free of obstructions.
No deficiencies being cited during today's inspection.
