Licensing Program Analyst (LPA) Cassandra Mikkelson arrived unannounced at the facility to conduct a case management visit. Licensee recently notified the Department via letter that facility was closing.
Facility does not have any residents currently in the home. Facility and property are now in the possession of property owner as property was leased to facility licensee.
Copy of today's report sent to licensee for signature as licensee was not present during visit.
