Facility Evaluation Report
On 10/14/2025, Licensing Program Analyst (LPA) Cheyenne Ratajczak arrived unannounced at the facility to conduct a case management visit regarding an incident report the department received on 10/13/2025. LPA met with Staff Karen Mutuku and explained the purpose of the visit.
The incident occurred on 10/13/2025 at approximately 7:20 a.m. Facility staff went to check on Resident #1 (R1) for a second time that morning. S1 observed R1 not in their room or anywhere in the facility. Staff had last seen R1 at 6:30 a.m. when they check on them the first time. R1 had been living in the facility for four (4) days. Interviews indicated R1 did not have any issues while residing at the facility the last couple of days. At this time R1 has not returned to the facility.
Exit interview conducted and a copy of the report provided to facility staff. Signature on these forms acknowledges receipt of these documents.
