Facility Evaluation Report
On 11/25/2025 Licensing Program Analyst (LPA) Cheyenne Ratajczak arrived at the facility unannounced to conduct a post licensing inspection. LPA met with Administrator Maylan Hill and explained the purpose of the visit.
LPA and Administrator conducted a tour of the interior and exterior of the facility. Areas toured include but not limited to: resident bedrooms, bathrooms, kitchen, garage, backyard and common areas. Resident bedrooms were properly furnished and maintained. Bathrooms were observed to be clean and sanitary. Facility food supply is in compliance with two (2) days of perishable and seven (7) days of non-perishable food items. Smoke and carbon monoxide detectors are operational. The fire extinguisher located right off the kitchen was serviced on 09/02/2025. All exits were unobstructed. All toxins, medications, and sharps were locked and stored away. Required postings are posted throughout the facility.
LPA reviewed two (2) resident files. Residents files have the required documents. LPA reviewed two (2) staff files. A review of staff records indicates that all facility staff has received criminal record clearances and/or are associated to this facility.
No deficiencies being cited during today's inspection.
Exit interview conducted and a copy of the report was left at the facility.
