The inspector’s account

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a case management inspection following receipt of an incident report. LPA met with DSP Lavenia Dokonivalu and DSP . LPA spoke to the Administrator, Annedreah Santiago, by phone. At the start of the inspection, LPA observed (3) clients in the common area and was advised (1) client was resting in their room.

LPA and the administrator discussed the incident report involving client (C1) on February 7, 2026 (10:00 am) in the common area. The administrator confirmed that (C1) returned to the facility yesterday, February 17, 2026 (4:00 pm). The incident report states that (C1) became upset with a friend who was visiting and exhibited several behavioral expressions. (C1's) friend immediately left and staff asked (C1) not to follow the friend. Local law enforcement arrived (10:25 am) and stated the friend had contacted them. (C1) left the facility for a medical evaluation, and (C1's) responsible persons, the Regional Center and CCLD were immediately contacted. The administrator contacted the hospital for an update on (C1's) status. (C1) did not have any medication changes other than changing the medication from (4) 100 mg tablets of Quetiapine to (1) 400 mg tablet of Quetiapine.

The administrator stated the other (3) clients were present at the facility during the incident and were "shaken up" so went in their rooms. The administrator arrived to the facility immediately following the incident. The administrator explained that prior to the incident, (C1) was receiving therapy services that were gradually discontinued due to (C1) showing improvement. The Regional Center is trying to get the services to resume, as (C1) feels they are needed. (C1) does not attend their day program consistently. LPA observed (C1) relaxing on the couch watching television. Staff are continuing to provide extra monitoring.
