The inspector’s account

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a case management inspection following receipt of an incident report. LPA met with Nilda Rockey, DSP and Cornellia Xayphone, DSP, and was advised the Administrator was out of the facility currently. LPA observed (4) of (4) clients present in the common area at the time of arrival and DSP staff to be completing tasks in the kitchen. One client (C2) left on an outing with a friend during the inspection.

LPA reviewed an incident report that was submitted to the Department on December 26, 2025 relating to an incident involving client (C1) and law enforcement on December 24, 2025. The report states that (C1) returned to the facility on December 25, 2025 (1:00 am) with the Administrator and is doing fine.

LPA spoke to (C1) during today's inspection. (C1) stated they is doing fine now. (C1) stated they went to day program today and will continue going. (C1) acknowledged that the Administrator discussed the incident with them.

LPA discussed the incident with both DSP's who were present today. They both acknowledged (C1) continues to participate in their daily routines without any issues, and the Regional Center has been involved, which the incident report states.

LPA observed the facility to be clean, in good repair. The administrator was notified that LPA conducted an inspection.

There were no deficiencies observed. There are no citations issued in this report.
