The inspector’s account

Licensing Program Analyst (LPA) Sabrina Calzada arrived at the unannounced to conduct a case management following receipt of an incident report on July 29, 2026. LPA met with met with Stephanie Howard, Program Director, and stated the reason for the inspection. The following was discussed:

LPA and Program Director discussed how client (C1) had a choking incident on July 29, 2026, while eating their lunch. Staff immediately responded and provided several "back blows" , which partially cleared (C1's) airways; however, (C1) still did not have their voice back entirely. 9-1-1 was contacted and arrived on site, was not able to take all of their vitals. so (C1) was taken to the Emergency Room for further medical evaluation due to possibly having more of the Cheeto/food stuck. An X-ray was done to determine there was nothing else stuck.

(C1) returned to the program the next day and was back to baseline.

There was no change in (C1's) diet type, and staff will continue to monitor (C1) closely during lunch/snack hours. Staff will be given more instruction this week on which clients to pay more attention to during feeding hours and how to respond, including with back blows, Heimlich maneuver and calling 9-1-1.

There are no deficiencies issued in this report.
