Facility Evaluation Report
Licensing Program Analyst (LPA) Sabrina Calzada arrived at the day program unannounced to conduct a case management inspection following receipt of an incident report on 1/8/2026. LPA met with Stephanie Howard, Administrator, and stated the reason for today's inspection. The following was discussed:
LPA and the administrator discussed an incident submitted to the Department on January 8, 2026 for an incident occurring on January 7, 2026 involving client (C1) missing snacks/lunch and medications. The administrator stated staff (S1) was focused on completing paperwork at the time of the incident and not aware they were to provide supervision to (4) clients, including ensuring lunch and medications were given. The administrator added that (C1) won't approach staff to inform them if they have missed a snack, lunch or medication and confirmed she was made aware that (C1) missed their snack, lunch and medications from (C1's) family member later on January 7, 2026. The administrator completed an initial staff training on January 8, 2026, specific to (C1) and agreed to conduct a follow up staff training for all clients who take medications, also ensuring all clients are offered their lunch, or a lunch made at program. Additionally, the administrator and (4-5) staff have set reminders on her phone when (C1) is to take food or medications. LPA attempted to talk to (C1) but (C1) was engaged in using their electronic device, but showed LPA where their lunch box is in the refrigerator, which confirmed morning snack and lunch/medications were taken.
Also discussed was client (C2) and specific incident reports leading up to (C2) discharged from the program due to agitation/behaviors towards staff/clients. All incidents were reported to the regional center but not to CCLD, due to no medical attention needed. LPA to obtain clarification and obtained paperwork on SIF/SIR.
