Facility Evaluation Report

Licensing Program Analyst (LPA) Melissa Parks arrived on Tuesday February 17, 2026, to conduct the unannounced annual inspection.

During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA reviewed resident (6) and staff (3) files. All resident files contained the required paperwork. All staff files contained the required paperwork and training.

LPA and Licensee Christine toured the facility together to ensure the health and safety of residents in care. The areas toured included resident bedrooms, bathrooms, kitchen, laundry room, and backyard. Facility water temperatures were within the required range. Facility had a fully stocked first aid kit. All required postings were observed. All chemicals and knives were kept locked and inaccessible to residents. Fire extinguishers had current inspection tags.

LPA obtained a copy of the current liability insurance.

LPA received the following documents to associate Maria Paz Carrasco as Administrator: administrator certificate, LIC 215,and LIC 508. Facility to send LPA LIC 501 and LIC 503 with TB results.

Exit interview conducted. No deficiencies cited. A copy of this report was provided to the facility.
