05/28/2026 and conducted by Evaluator Kerry Hiratsuka

Staff are not properly trained on transfers

Licensing Program Analyst Hiratsuka, conducted this unannounced complaint visit.

LPA toured the facility and interviewed staff. LPA reviewed staff training. The staff have the required training for transfering residents.

Based on the above, the allegaion is unfounded.

A finding that an allegation is UNFOUNDED means that the allegation is false, could not have happened, and/or is without a reasonable basis.
