Complaint Investigation Report
On 11/30/2022, LPA Michael Hood arrived at the facility and conducted a medication count. LPA observed a loose pill belonging to resident (R1) fall from an open bubble pack that was already administered. The pill was observed to be an 8:00 AM 10mg Loratadine.
A Facility Action Report (FAR) was generated by Alta California Regional Center (ACRC) on 12/20/2022 to address violations per Title 17 regulations. Information on the FAR is as follows:
On 12/12/2022, ACRC Liaison Support Coordinator (LSC) conducted an unannounced visit at People’s Care Winfin. Upon review of resident (R2’s) medications and Medication Administration Record (MAR) dated 11/18/2022 to 12/15/2022, ACRC observed missing staff initials/signatures for two different medications.
Based on records reviewed, the preponderance of evidence standards have been met. Therefore, the above allegation is found to be SUBSTANTIATED. Per California Code of Regulations, Title 22, Division 6, Chapter 8, a deficiency is being cited on the attached 9099-D.
Exit interview was conducted with House Lead. A copy of this report and appeal rights were provided. The House Lead’s signature on these forms acknowledges receipt of these documents.
