Facility Evaluation Report
Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct an annual inspection. LPA met with staff and explained the purpose of the visit. LPA waited for Administrator's arrival to start the inspection. Upon LPA's arrival, LPA wore a surgical mask to ensure the health and safety of clients in care.
Today's census is five clients in care, facility is licensed for five non-ambulatory clients, which five may be bedridden.
LPA observed Administrator Certificate #6070896735 to be posted in the common area, Administrator Certificate was observed to be active with expiration date of June 6, 2026. LPA observed facility to be adequately staffed with the presence of one registered nurse, two direct support professionals, and one respiratory therapist.
Today's visit, LPA conducted a tour of the facility. Areas toured included but not limited to: clients bedrooms, bathrooms, laundry room, kitchen, and the common areas. LPA observed clients to be in their private rooms, LPA observed medications to be centrally stored in a locked drawer. LPA observed fire extinguisher to be recently serviced with service date of May 14, 2025. LPA observed cleaning supplies to be stored separately from food supplies. LPA observed food to be properly stored and protected from contamination. LPA observed dishes and utensils to be clean and in a safe condition. LPA observed facility to be clean, sanitary and in good repair. No concerns reported at the facility.
Emergency Disaster Plan was observed and reviewed. LPA completed the CARE tool and observed facility to be in compliance. No deficiencies cited.
Exit interview and a copy of the report was provided.
