On 7/11/23, Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management visit while delivering complaint findings and met with Executive Director (ED).
On 7/10/23, the department received an incident report of resident theft or loss.
LPA was informed by ED that an internal investigation has been conducted and a caregiver suspected has been suspended. The report was given to local police as well.
LPA interviewed R1. LPA also received records regarding S1.
As a result of today’s inspection, no deficiencies were noted at this time.
