Facility Evaluation Report

Licensing Program Analyst (LPA) Graham Gunby arrived on 03/05/2025 to conduct an unannounced annual inspection. LPA rang the doorbell in an attempt to see if anyone was present at the facility and if there were any residents under care and supervision at this time. Several attempts were made with the doorbell but LPA was unable to get anyone to come to the door. LPA made contact with Administrator Lendour, and scheduled a visit to the facility on 03/25/2025.

LPA observed no residents at the facility. Areas toured included but not limited to: private residents room, bathrooms, kitchen, backyard, storage area, and common areas. LPA observed the fire extinguisher in the dinning room. LPA observed sharps, toxins, and medications to be locked and secured. LPA reviewed Administrators file containing all the required paperwork and training.

No deficiencies cited. Exit interview conducted. A copy of this report was emailed to the Administrator.
